The corticosteroid Triamcinolone Acetonide included in Dicot Cream is a member of this class. It is used successfully to treat a variety of inflammatory skin conditions. Indication of Dicot Cream

Dicot Cream is used for the treatment of -

  • Eczema refers to a skin condition where patches of skin become inflamed, itchy, cracked, and rough. This can lead to a rash on the arms and behind the knees and can be triggered by a variety of factors such as foods, drinks, skin products, soaps, chemicals, detergents, makeup products, chlorinated pools, and stress.
  • Psoriasis, on the other hand, is a skin disorder in which skin cells multiply, resulting in bumpy, red patches covered with white scales. Symptoms of psoriasis include red patches on the skin, dry and cracked skin, swollen joints, and an itchy sensation.
  • Sub-acute cutaneous lupus erythematosus is an autoimmune disease that affects the skin, characterized by red, ring-shaped, or raised lesions on the neck, back, or chest. This condition may be caused by genetic or environmental factors, including ultraviolet radiation, medications, tobacco use, and infections.
Pharmacology

Econazole Nitrate: Athlete's foot, jock itch, and ringworm are just a few of the fungal skin illnesses that are treated with econazole. Additionally, pityriasis (tinea versicolor), a fungal infection that results in lightening or darkening of the skin on the neck, chest, arms, or legs, is treated with this medicine. Econazole is an azole antifungal that functions by inhibiting fungus development.

Triamcinolone has mainly glucocorticoid activity. It suppresses the migration of polymorphonuclear leukocytes and reduces capillary permeability thereby decreasing inflammation.

Dosage & Administration of Dicot Cream
  • For adults, the cream should only be applied twice daily, in small amounts to the affected area, once in the morning and once in the evening. Do not use an occlusive dressing or apply to large areas of the body. The treatment should be continued until the symptoms of inflammation subside, but not longer than 2 weeks. If treatment is needed beyond 2 weeks, a preparation containing only econazole or econazole nitrate should be used.
  • For pediatric patients, there is a higher risk of developing HPA axis suppression and Cushing's syndrome due to their larger skin surface area to body weight ratio. Caution should be taken when using the cream on pediatric patients.
  • Apply Dicot Cream as directed by your physician. It is for external use only. Wash your hands with soap and water and apply a thin layer of the medicine to the affected area of the skin and rub it gently. Do not apply it to or near the eyes. Do not wrap or cover the skin with a bandage unless advised by the doctor.
Interaction

Econazole is an antifungal medication that is metabolized by the CYP3A4/2C9 enzyme. Triamcinolone can reduce the amount of salicylates in the blood. NSAIDs increase the risk of gastrointestinal bleeding and ulceration, and can also antagonize the blood glucose-lowering effects of antidiabetic medications. Amphotericin B, beta-blockers, potassium-depleting diuretics, and theophylline all increase the risk of hyperkalemia. With drugs like ciclosporin, carbamazepine, phenytoin, barbiturates, and rifampicin, triamcinolone clearance is increased.

Contraindications

This medication should not be used to treat tuberculous lesions or viral skin diseases such as herpes, vaccinia, and varicella. It should also not be used in individuals who have a true hypersensitivity to the medication.

Pregnancy & Lactation

During pregnancy, the use of topical corticosteroids containing Triamcinolone Acetonide should be avoided, as it can penetrate the placenta and potentially cause fetal developmental problems. The impact of this finding on human pregnancy has not been fully determined. The use of high doses of topical steroids or prolonged use of topical steroids during pregnancy is not recommended.

A small amount of Econazole and Triamcinolone may be secreted in breast milk, so this cream should not be used by nursing mothers, or if it is used, breastfeeding should be discontinued during treatment.

Precaution & Warnings
  • This cream is meant for external use only and should not be applied to the eyes or mouth. If any signs of hypersensitivity or chemical irritation occur, the use of the drug should be discontinued immediately.
  • Topical corticosteroids can be absorbed in large amounts and have systemic effects, such as adrenal suppression. Treatment over a large skin surface area, application to damaged skin, use of occlusive dressings, and prolonged treatment duration may increase systemic absorption.
  • Skin thinning, atrophy, striae, telangiectasia, and purpura have been associated with the use of topical corticosteroids.
  • Extra caution is necessary when using this cream on children due to their thinner skin and larger skin surface area relative to body weight, which can lead to higher corticosteroid absorption. This cream should only be applied to small areas and for short periods of time (less than 2 weeks) and not more than 10 percent of the body surface area.
